What caused it?

Just like ordinary acne, there are many potential causes of mask acne: infrequent cleaning of the mask; cleaning with strong detergent; irritation caused by your mask material, or how often you must wear it.

Masks help prevent saliva and any other droplets (expelled from breathing, talking, coughing, or sneezing) from entering the air and possibly spreading Covid-19 (or other diseases).
